Information required on a Civil Defence Form

To complete a civil defence form, you will be required to provide:
Names: You are expected to enter your legal names as contained in your birth certificate or marriage certificate, in the case of a married woman.
Date of birth: Enter accurate details of your date of birth in the space provided on the form. Make sure to follow the format indicated (DD/MM/YYYY, MM/DD/YYYY or YYYY/MM/DD) when providing this information.
Telephone Number: In the space provided, you will be required to enter a telephone number that you can be reached at, preferably yours. This may be your official number, residential number or both.
Address: This can be your permanent, residential address or both. Also, you may be required to provide your office address if you have a job.
Nationality: You are expected to enter details of your nationality- country of birth or naturalisation.
Previous experience: This part of the form is included to know if you have worked with the civil defence organisation or have an affiliation with it.
Identification Mark: This part of the civil defence requires you to list any mark or scar on your body. This could be a birthmark or a scar that is prominent and can be used to identify you.
Photograph: You are required to affix a passport-sized photograph of yourself to your civil defence form before submitting it.
Name and Address of Present Employer: This part is included in the form to know if you currently have a job. If yes, you will need to include both the name and address of the employer or company you work for.
Academic Qualification: You are expected to provide accurate information about your educational background, qualifications, and certification in the space provided.
Self Declaration: You are required to sign a self-declaration indicating that you are who you claim to be and the information you have provided is accurate and true.
How to Fill a Civil Defence Form

Box 1
Enter your names in full in the box provided. You can enter them in block letters.
Box 2
Enter your father’s or husband’s name. For females, this is similar to entering your maiden name.

Fill in your date of birth. A common format to use when completing this part is the DD/MM/YYYY.

You are expected to enter your nationality.
Box 5
If you have a mark, maybe a birthmark, tattoo or a scar on any part of your body, you should indicate this in the space provided. Also include what part of your body the mark is on.
Box 6
Enter details of your address; both present and permanent addresses.
Box 7
State your occupation and the address of your office if you are employed or run a business.
Box 8
Write out both your office (if employed or a business owner) and residential phone numbers in the spaces provided.
Box 9
This part of the form only applies to people under corporate employment or individuals with a named employer. You'll be required to enter the name and address of your employer in the spaces provided.
Box 10
This part of the civil defence form requires you to affix a recent passport-sized photograph of yourself.
